Smith obtained the fake prescription while working for a Wheaton Franciscan Health Group medical clinic in Glendale and took it to the pharmacy to get 40 Vicodin pills. An internal investigation by the medical group discovered the fraud and alerted Glendale police.

She was located and arrested during a traffic stop where she had Hydrocodone pills and marijuana in her car. Smith admitted to the fraud, saying she has an addiction to Vicodin.
